Safe and Effective
Spinal manipulative therapy (SMT)—the hands-on adjustment most associated with chiropractic care—has its strongest clinical support for low back pain. In a JAMA meta-analysis of 26 randomized trials, SMT was linked to statistically significant short-term gains in pain (about 10 points on a 100-point scale) and function for acute low back pain lasting six weeks or less, with no serious adverse events reported in the included trials (Paige et al., 2017). For chronic low back pain, systematic reviews have found evidence that manipulation and mobilization can reduce pain and disability compared with other active treatments such as exercise or physical therapy, with thrust manipulation often showing a larger effect than mobilization alone (Coulter et al., 2018). A 2019 BMJ review of 47 trials similarly concluded that SMT produces effects comparable to other recommended conservative therapies for pain, with a small additional benefit for function in the short term (Rubinstein et al., 2019).
The Science of Chiropractic
Quality of Life
Systematic reviews of randomized trials indicate that cervical manipulation and mobilization can reduce pain intensity and neck-related disability, particularly in the short term and when used as part of a broader plan that includes exercise. One 2023 meta-analysis of 17 trials in people with chronic neck pain found meaningful reductions in pain and disability versus control interventions, with adverse events generally limited to temporary soreness (Complementary Therapies in Clinical Practice, 2023). Separate reviews of acute neck pain have likewise reported improvements in pain and cervical range of motion after SMT
Benefits Large and Small
Research concerns how an adjustment may change pain, not just whether patients feel better. Experimental and review work shows that a high-velocity thrust stimulates muscle spindles and other paraspinal mechanoreceptors, producing a brief burst of sensory input into the spinal cord (Pickar and colleagues; reviewed in Gevers-Montoro et al., 2021). That input appears to dampen nociceptive signaling at the segmental level, raise pressure-pain thresholds, and engage descending inhibitory pathways from the brain—mechanisms that help explain short-term hypoalgesia even when lasting structural “realignment” is not the main story. A 2025 living review of mechanistic studies supported peripheral, spinal, and supraspinal responses after manual therapy, while noting that the quality of that mechanistic evidence ranges from low to moderate and that placebo and contextual effects still need better controls. In short, the science supports SMT as one conservative tool for common spine pain, acting through both modest clinical benefit and measurable changes in how the nervous system processes pain.
